Certified Medical Model
The Caring Connection Adult Day Health Center is a certified medical model Adult Day Center. The program has passed a stringent set of standards provided by the Connecticut Association of Adult Day Centers and the State Department of Social Services for the State of the Connecticut. Other adult day centers in the state may be "Social" models and provide no professional medical staff. Nursing services are included in the daily rate (excluding medications, special medical equipment, or supplies).
What Nursing Services does the Caring Connection provide?
All health maintenance, including:
- Recording blood pressures, weights, and pulses monthly or as ordered
- Assistance with ambulation and range of motion
- Care of incontinence
- Wound care and dressing changes
- Medication dispensing and management
- Assistance with set up of medical appointment
- Emergency care
- Blood sugar testing, as needed
- Assistance with special diets as eating
- Behavior modifications
- Updates with caregivers, case managers, and appropriate medical personnel
- Personal care
- Flu shots/PPD's (test for tuberculosis)
The interdisciplinary team is responsible for devising an individualized care plan designed with input from medical professionals, caregivers and family to meet the needs of each client. The goal is to maximize independence and keep clients save, active, and participating in their homes and the community.
The Nursing Department also provides a serviced called "Pedicare", an outside agency that provides foot care by a Registered Nurse at the Caring Connection facility. There is a fee for this service.
What if I need therapy, have a special diet, or have medical appointments?
Speech, Occupational and Physical Therapy appoinbtments must be arranged through you personal physician and rehab centers. Clients may be transported to and from the center during the day by caregivers and transportation services for any appointments. For medical appointments, clients may be picked up and dropped off at our center as to assure not to miss an entire attendance day. Please notify the staff of any changes in your schedules.
Special diets are a part of our program, including: calorie restrictions, food allergies, no salt added, no concentrated sweets, diabetic diets, and /or soft or modified consistencies. Two snacks are prepared daily by the Caring Connection Staff and a full nutritious hot lunch is provided to the Caring Connection through the COmmunity Renewal Team.
All professional nursing staff members are licensed with the State of Connecticut.
Because the Caring Connection is certified by the Connecticut Association of Adult Day Centers and the Department of Social Services for the State of Connecticut, the programs has contracts with the Connecticut Home Care for Elders, Connecticut Community Care, Inc. the Veteran's Administration, and the North Central Area Agency on Aging for provision of medical model services.
