Information needed for a certificate of Insurance or Evidence of Insurance from the Town

At times, some of you will need a certificate of insurance (COI) or evidence of insurance. Certificates of Insurance and evidence of insurance is not generated from the Risk Manager’s office. It must be requested by the Risk Manager from the town’s Broker and their staff then puts together the information requested and sends it back to the Risk Manager’s office and to the requesting agency.

To assist you, the following is the information needed in order to fulfill a request:

  • Grant or contract number, or some identifying title
  • Name of requesting agency or vendor
  • Address of requesting agency
  • Telephone number, fax number of requesting agency
  • Name of contact
  • Specific coverages being requested (most times it is general liability, workers comp, property, auto).

Once the COI has been issued, it is automatically sent to the requesting agency and to the Risk Manager’s office. It will then be sent to the individual requesting it so it can be attached to their application. Generally it takes a day or two for the request to be made and fulfilled. Please allow enough time for the request to be made, reviewed and submitted.
